My mother was recently in care here, and had a wonderful time. The staff was kind, loving, and extremely attentive, administration was communicative, and the food was delicious. The facility was clean and has lovely church service that really helped her emotionally.

I am a patient sent from el Camino hospital. I had a good stay at Sunnyvale post, the staff was attentive & I was able to go home after therapy & my nursing care. I give the service 10/10

I am a returning patient at Sunnyvale post by request. My first visit was a success with physical therapy & staff being so helpful. I made it back home successfully. This visit I was at Stanford hospital and requested to return because it was a familiar facility & I knew I would get the best of care. I especially like the admissions team Jessica & Rayleen.

Me and my Family have experienced the FINEST and most CARING of souls here at Sunnyvale Post-Acute Center! My Dad was here a year ago, and my Mum just recently. Both of them were brought back into action of every day life by the wonderful skilled nursing staff here! It was an absolute pleasure to have gotten to know the wonderful staff here – they are all very personable, caring and skilled. I always thought I was a ‘fairly’ good-person – But these guys and gals take the biscuit!! – I BOW DOWN to all of them – they work so very hard here to make a difference in people’s lives! I’ve seen the staff here SACRIFICE their own personal time – lunch times, breaks, days off, ‘after work’ plans – just so they can take care of their patient’s needs… (they don’t even let the patients/visitors know this due to your kind, humble and professional natures) – but when it comes to ‘human kindness’ – my RADAR goes off and I notice even more of their amazing kindness!! I’ve also seen them jump into action and support each other as a TEAM too! Loved seeing how well they all get along so famously – Excellent teamwork! The SINCERE and heart-felt COMPASSION that they all demonstrate via their ‘NATURAL way of being’ – through their ACTIONS, their WORDS, their PATIENCE, their SKILLS, their LOVE – is a lesson in ‘human-kindness’ to us all! Personally, they ALL taught me SO MUCH about how to ‘handle’ challenging situations (especially ones where I might sometimes fly-off the handle because I don’t know how to respond to awkward situations – due to my ego, patience & lack of ‘know how’) – They all taught me how to handle it with LOVE and KINDNESS just by their behaviors and interactions! I’m just ‘one’ example of the lives they’ve made BETTER and I THANK them for LEADING the way when it comes to human kindness! They rock at that! Being as wonderful as they are takes EFFORT, some of it is learnt through experience (the rest of it comes naturally to them) – Sometimes this effort they’ve accomplished ‘solo’, sometimes it might take a ‘village’, but at the end of the day, it takes ‘THEM’! Continue the GOODNESS like you ALL do!! I look up to the entire staff here at Sunnyvale Post-Acute Center (we ALL do – Patients, Family, Visitors and Strangers too). The place is always clean, from the resident rooms to the common areas and hallways, which makes a big difference and gives peace of mind. Staff were also quick to respond whenever something was needed. Social Services was especially helpful in guiding us through programs we qualified for after discharging home, since those applications can be confusing to navigate. The outside of the building is well kept too. The grounds are neat, the entrance looks welcoming, and overall we were very happy with our stay at Sunnyvale Post.

My experience with Sunnyvale post acute center is very positive. I was a resident there for 17 days last year, and my father was a resident there this year for a period of three weeks. The staff at the facility was wonderful, very attentive and able to take care of all of our needs working with the administration departments, social services, and admissions were very helpful in guiding us through the whole process. The physical therapy and occupational therapy team was exceptional. They provided the best care we could ever hope for. After my stay at the facility, my father fell ill and required skilled nursing. Sunnyvale post acute center came up on the possible list of places we can place my father. We applied right away because I knew the type of service that they provide would be the best. It was for me. I recommend this facility to anyone who requires skilled care and a positive experience.

Jessica is true pro and Jen the head nurse is the best nurse I have ever met and the facility is spotless you can eat off the floors I hope when it’s time To go I am there

My husband has been here several weeks now and has been making progress toward regaining enough mobility to come home hopefully soon. The staff have been top notch in caring for him, arranging transportation to medical appointments and in every other way. And everyone has been super responsive to any concerns we expressed.

Night time crew are mostly lazy sitting chairs watching their smart phones. I asked blanket to the guy sitting chair answering “I’m not your nurse ” Asked another nurse, it was 5 steps from the guy’s chair. Patients staying here are difficult to speak so nurses don’t have the responsibility to patients. I was transferred from Hospital , nurses qualify are big difference . Some Assistant nurses are not nurse like security or janitor. But some of nurses are very good.

Posting on behalf of my wife: My dad has spent almost 3 months at Sunnyvale Post-Acute after his hospitalization.  He received a ton of care there: Physical therapy, Occupational Therapy, Speech Therapy (including Swallowing), and even music & aromatherapy.  He has improved to the point that we can bring him home soon.  The primary doctor saw my dad once a week, and the doctor of rehab saw my dad twice a week.  Then there were so many nurses, from RNs to LVNs to CNAs (it is a skilled nursing facility after all)!  They also individualize the meal plan per the hospital discharge orders.  For my dad it was low-carb vegetarian, and pureed food only, with 1/1 feeding assistance.  They also crushed medicines.  The staff was amazing: not only are they highly skilled, but they have a lot of empathy towards the patient.  They even got to know the family, as my mom & I were there a lot.  We also enjoyed meeting the nursing students.  A group of the them came to my dad’s room to sing Happy Birthday on his 90th birthday! This was our first experience with a SNF, and should we need one again, we would request this one!

I am a patient here and everything went really well. I am getting discharge and it is hard to say bye to everyone. From the nurses to the therapy staff, everyone has been so helpful. The therapy team helped me get back on my feet and were very patient with me. The facility is very clean and odor free. I will miss the staff here.

My mother recently completed her stay at Sunnyvale Post Acute, and I cannot speak highly enough about the outstanding care she received from this incredible team. From the moment we arrived, Jessica in Admissions made what could have been an overwhelming process feel completely manageable. She explained every step of the intake process, helped us understand what to expect, and ensured all paperwork and assessments were handled smoothly. What truly stood out was how she was always on top of everything – proactively reaching out to check if there was anything she could do to help our family during the transition. Angelica, our Kaiser case manager, went above and beyond to ensure my mother’s case was handled with utmost care and diligence. She expertly bridged insurance requirements with my mother’s care needs, advocated with insurance, ensured coverage for necessary services, and helped us understand available benefits. Her dedication to getting every detail right was exceptional. Kaiser is lucky to have her. Renee, our RN, handled the crucial transition from hospital to facility with exceptional thoroughness and attention to detail. She ensured all the complicated medical information was transferred correctly and nothing was overlooked during this critical handoff. The daily nursing care was provided by a dedicated team of RNs who monitored medications carefully, kept our family informed about medical status and progress, and were always available to answer questions about the treatment plan. The clinical expertise of the entire nursing staff gave us tremendous peace of mind. The CNAs, including Alvin and Maheen, provided daily hands-on care with incredible patience and kindness. They assisted with personal care and mobility while treating my mother with dignity and respect every single day. Their warmth and caring nature made such an impact that my mother ended up considering them part of her family! I know there were other CNAs who cared for my mother during different shifts, and I’m grateful to each of them as well. The therapy team – Gargee, Kaavya, and Joe – designed individualized PT/OT programs that were key to my mother’s recovery. They worked patiently to rebuild her strength and mobility, taught safe techniques for daily activities, and celebrated every milestone toward independence. What made Gargee and Kaavya especially wonderful was how they found creative ways to keep my mother occupied even during off days – suggesting biking exercises or activities she could do in bed. The special relationships my mother formed with them made therapy something she actually looked forward to. Dr. Rao and Dr. Lively provided excellent medical oversight throughout my mother’s stay. Dr. Rao’s thoroughness was remarkable – she maintained constant communication with our family through emails and phone calls, treating my mother as if she were her own family member. Dr. Lively brought such empathy to her care, spending generous amounts of time with each patient and approaching every interaction with utmost care and thoughtfulness. What truly set Dr. Lively apart was that she not only focused on making the patient better, but fully understood the caregivers’ needs and addressed them with anticipation. Both doctors carefully monitored her condition, adjusted treatments as needed, and communicated clearly with our family about progress. Matthew, the social worker, was invaluable in discharge planning. He connected us with community resources and helped navigate the complex process of ensuring my mother had everything needed for a successful transition home. And Revathy at the front desk deserves special recognition for being such a warm, welcoming presence throughout our experience – her friendly face and efficient coordination made every visit feel more comfortable. If you’re looking for skilled nursing care that combines clinical excellence with genuine compassion, I highly recommend Sunnyvale Post Acute. This team truly made all the difference in my mother’s recovery journey.

My husband was at this facility for almost 8 weeks recovering from a heart attack and major heart surgery. This was a difficult time for our family and I was thankful to meet Jessica when I visited the facility for a tour. The place smelled clean and the staff were friendly. I had visited a post-acute facility prior and it was HORRIBLE. Jessica made me feel comfortable and at ease to have my husband continue his care at SPAC. During his stay the CNA’s & Nurses were helpful. It was not always smooth; however, once we voiced our concerns and needs, they responded quickly. Angelica is a good Case Manager. Thanks so much Angelica for all your help and guidance. The Physical Therapy Team (GG, KK, Angie, Brandon, Camille, Jason, Joe and all the others) are AMAZING, a 10++++++ and everyone is very nice and helpful. They know what they are doing!! We cannot thank them enough for all their help and teaching us everything they did over the 8 weeks. We recommend this facility for post-acute care. Thanks again to everyone that assisted in the recovery process.

I am very happy with the staff and the facility. My mom stayed almost 3 weeks there and she loved the place. PT/OT people were very supportive too. Few time some unpleasant experience happened during night time (with night staff )but overall it was very good experience.

Hello! I just wanted to make comment about all the CNAs especially Jamuna CNA. She spent about 40 min getting the knots out of my hair to braid it when I got here. She fed me, and gave me a shower. As far as a Representative of Sunnyvale post Acute Center she has really given me an excellent impression of the facility and some of the necessary functions it provides customers. As a customer I have all positive experiences including far above what my expectations were when I arrived. I am so happy that things worked out the way they did as soon as I arrived. I am impressed with the kind of help I received as soon as I arrived because i really needed it ! Thank you .

If I could give this place zero stars I would! My brother had fallen and broke his hip and after his surgery he was sent to SPAC. He was improving slowly but in January he started choking and was rushed to El Camino Hospital where he was diagnosed with pneumonia. On February 8th he was sent back to Sunnyvale Post Accute Center. On Monday March 10th, he called our mom at 5:15 in the morning, pleading with her to pick him up and bring him home because he couldn’t breathe. Mom called and when she didn’t get an answer we drove there to find out if he was okay. When we got there there we several nurses standing around but the horrific situation we saw when we got to his room will forever be etched in my head. Mt brother was unresponsive and the nurse who just found him with his eyes rolling to the back of his head and foaming at the mouth was trying the revive him. We tried to call his name to get him to wake up but nothing. The nurse was also putting a hose down his throat to clear his airwave and we were asking if the had called 911 they said they had called several minutes ago. I then started to ask why the staff was just standing around and not helping and why the ambulance wasn’t there yet I got no response. The first responders finally got there and they started to try to revive him, thank god he still had a pulse and was still breathing. Once we got him to the hospital and they were able to assess him we found out that he had pneumonia again plus he had a bladder infection as well as Rhino Virus. This place needs to be shut down before they actually kill someone. PLEASE DO NOT TAKE YOUR LOVED ONES THERE.
