Nursing Services
Sunnyvale is committed to providing industry-leading care to each of its residents and has instituted innovative programs to care for their needs. Specifically, Sunnyvale has invested in a respiratory therapist and instituted its own respiratory program catered towards the large populace of respiratory patients. Sunnyvale is also the only provider of peritoneal dialysis in Northern California.
Sunnyvale has instituted a High-Frequency Rounding Model in our facility which entails a physician, Dr. Matthew Gillett, rounding in the facility 3-4 times per week on each of our short-term residents. The physician also oversees a physician’s assistant that is present in the facility on a daily basis with the intention of spotting signs and symptoms of deterioration earlier to adequately adjust the care and avoid readmission to the acute hospital.

- Onsite Medical Director coordinates and improves medical services 3 to 4 times a week

Alzheimer’s Care
Our nurses work with the client’s family to create a comfortable environment including simple routines and visual cues as a positive substitute for diminishing mental processes. We do not have a specialized unit for dementia and Alzheimer’s care. We do accept patients on a case by case basis. Please contact us to learn more.
Diabetes Management
Our experienced nurses provide help with all aspects of managing diabetes including:
- Blood glucose monitoring
- Insulin administration and tracking
- Exercise planning
- Menu planning, education and more.
Restorative Nursing Program
At Sunnyvale Post-Acute Center we seek to maximize the quality of life for our long-term care population, who require a specialized restorative program. Of course, this service is included in the room and board fee.
Our therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:
- Ambulation
- Range of motion
- Activities of daily living
- Positioning and alignment
- Skin integrity and wound management
- Contracture management
- Bowel and bladder training
- Rehabilitative and social dining
- Strengthening and restraint reduction
- Weekly and monthly height and weight monitoring
Dietary Program
Our dietary program is designed to meet the dietary and physician-ordered needs of each resident in our care.
To ensure that each resident is receiving the proper nutritional needs we offer:
- Meals to meet specific dietary and therapeutic needs, according to physician orders
- Oral supplemental, as prescribed by each physician
- Regular nutritional assessments from our registered dietitian
